In order to properly assess your oral health, we require a brief health history. If you are a new patient and would like to print and complete this form, you can save some time by presenting it to reception when you arrive for your first appointment. Otherwise, we will provide you with this form on arrival.

We want to know basic information on your past dental history. This will allow us to offer you the best services and cater treatment options to fit your needs.

Transfer of Patient Records Consent Form

Would you like to have your dental records sent from another office?  Please print this form, fill out your name and the date, check the appropriate boxes, sign the bottom and present it to your previous Dentist's office.  We prefer to receive files electronically, however when this is not possible we will also accept files by fax and by mail.
